Prediabetes Diet

If your doctor says your bloodwork reveals prediabetes, you’re bound to have questions about what’s best to eat. People with prediabetes have fasting blood sugar levels that are elevated, but not to the point that they meet the criteria of type 2 diabetes.

Diet for Prediabetes — What foods should I eat?

A balanced diet is key to preventing prediabetes from progressing to type 2 diabetes. Rather than eliminating entire food groups or following fad diets, focus on healthy, sustainable eating habits that support long-term blood sugar control.

With that in mind, the Mediterranean diet is recommended for people with prediabetes, with its emphasis on whole grains, lean protein and healthy fats.

Other recommended resources include the diabetes meal planning recommendations from the Centers for Disease Control and Prevention and a version of the MyPlate guidelines issued by the U.S. Department of Agriculture and adapted by the American Diabetes Association (ADA).

The ADA’s version of the recommendations suggests these proportions for meals:

  • 50% of the plate filled with non-starchy vegetables, such as leafy greens
  • 25% with healthy carbohydrates, such as whole grains like brown rice, farro or quinoa
  • 25% with lean protein, such as chicken, turkey, fish or tofu
  • Incorporate fiber to reach a goal of 25 to 30 grams per day by eating a variety of fruits, vegetables and whole grains

Foods to Limit if You Have Prediabetes

In general, many nutrition experts highlight certain foods that tend to spike blood sugar more quickly and are worth limiting. These include:

  • Sugary drinks such as soda, sweetened teas, energy drinks, and fruit juices
  • Refined carbohydrates like white bread, white rice, pastries, and regular pasta
  • Highly processed snacks including chips, crackers and packaged baked goods
  • Foods high in added sugars such as candy, desserts and sweetened cereals
  • Fried foods that are high in saturated fats and can worsen insulin resistance
  • Limit saturated fats like fatty cuts of meat, butter and full-fat diary to support insulin sensitivity and instead have healthy unsaturated fats, such as avocados, nuts, seeds, canola and extra virgin olive oil. 

Prediabetes and sugar — How much is too much?

Carbohydrates are the body’s main source of energy. Glucose (blood sugar) fuels every cell in the body and is the brain’s preferred fuel. While your body needs glucose to function, eating too much added sugar and refined carbohydrates can raise your blood sugar levels and worsen your diabetes.

Blood sugar levels are key indicators of prediabetes, so it’s important to be aware of hidden sources of sugar. Added sugars can be found in many processed foods, including breakfast cereals, frozen meals, protein bars, sauces and salad dressings.

Recommendations for Limiting Sugar

The World Health Organization (WHO) recommends limiting added sugar to less than 10% of your daily calorie intake and ideally to 5% or less for additional health benefits. For someone following a 2,000-calorie diet, this means consuming no more than 50 grams (about 12 teaspoons) of added sugar per day, with an ideal target of 25 grams (about 6 teaspoons). 

The American Heart Association recommends even lower limits, advising women to consume no more than 25 grams (6 teaspoons) of added sugar daily and men no more than 36 grams (9 teaspoons) per day.

Practical Tips for Limiting Sugar

  • Choose water or unsweetened beverages instead of soda, sweet tea, energy drinks, and other sugary drinks.
  • Read food labels and look for added sugars in packaged foods, snacks, and beverages.
  • Eat whole fruits instead of candy, cookies, pastries, or other sugary desserts.
  • Choose balanced meals and snacks with protein, fibre and healthy fats to help reduce sugar cravings.
  • Limit highly processed foods that often contain hidden sugars and opt for whole, minimally processed foods whenever possible.

What should I eat for breakfast if I have prediabetes?

Choose foods that have lean protein, healthy fats, and high fibre carbohydrates. For example: Oatmeal topped with berries and pumpkin seeds with 1% milk, offer a well-balanced meal.

What fruits should I avoid with prediabetes?

Whole fruits can be part of healthy eating for people with prediabetes because they provide fibre, vitamins and antioxidants. Aim to choose whole fruits instead of fruit juice, which can raise blood sugar quickly. Consult with your healthcare provider regarding grapefruit as it can interact with certain medications.

How to Reverse Prediabetes — Lifestyle

Research studies such as the National Institutes of Health’s Diabetes Prevention Program Outcome Study (DPP) has consistently shown that healthy eating, regular physical activity, modest weight loss can reduce your risk of developing type 2 diabetes by 34% for four years compared to placebo, which was an outcome of the Johns Hopkins’ diabetes prevention program.

Stay active

Physical activity can help prevent diabetes while boosting heart health. People with prediabetes should try to get at least 150 minutes per week of moderate to vigorous exercise or aim for 10,000 daily steps. Be sure to check with your doctor before starting exercise or levelling up your current physical activity.

Small Weight Loss, Big Health Benefits

Even small amounts of weight loss can have a benefit. The ADA states that moderate rate reduction of 5% to 10% of your body weight can significantly lower your A1C level. 

Get Enough Sleep

The relationship between sleep, diabetes, and weight gain is important to understand. Sleep deprivation has been shown to increase people’s cravings for sugary foods, so people with prediabetes should make sure they’re getting seven to eight hours of sleep a night.

Limit Alcohol and Avoid All Tobacco

Limiting alcohol can help improve blood sugar control and reduce the risk of progressing from prediabetes to type 2 diabetes. Alcohol recommendations are no more than one drink per day for women and no more than two per day for men. 

Avoiding all tobacco products is also important, as smoking can increase insulin resistance and raise the risk of diabetes-related health complications.

Stay on Top of Your Numbers

People with prediabetes should be vigilant about their laboratory test results. If you’re at risk or have prediabetes, make sure you follow up with your A1C level with a blood test at least yearly.

Dietary Supplements and Prediabetes

Be cautious with dietary supplements. Some supplements may affect blood sugar and interact with diabetes medication. For example, ginseng, gingko, and garlic have been studied for their potential effects on blood glucose, but the evidence is mixed. In high doses—particularly when taken as concentrated supplements rather than as foods—they may increase the risk of low blood sugar (hypoglycaemia) in people taking diabetes medications.

Before starting any vitamin, mineral, herbal, or botanical supplement, talk with your physician, pharmacist, or registered dietitian to ensure it is safe, effective, and appropriate for your individual health needs and medications.

Work With a Dietitian if You Have Prediabetes 

Because many people with prediabetes may have other health conditions, such as high cholesterol or high blood pressure, it’s important to individualize nutrition recommendations. 

A registered dietitian can help develop a personalized nutrition plan that fits your lifestyle, improves blood sugar control, and reduces your risk of developing type 2 diabetes. Ask your primary care provider for a referral to a registered dietitian or check with your health insurance plan to see if nutrition counselling for prediabetes is covered.

Reversing Prediabetes — Is it possible?

A diagnosis of prediabetes does not mean that developing type 2 diabetes is inevitable. Healthy lifestyle changes—including eating a balanced diet, being physically active, maintaining a healthy weight, getting adequate sleep and managing stress—can help improve blood sugar levels and may prevent or delay the progression to type 2 diabetes. In many cases, these changes can even help return blood sugar levels to the normal range.

Healthy lifestyle is key to diabetes prevention and management

In observance of Diabetes Awareness Week, we are reminded that diabetes has quietly become one of the country’s most serious public health threats. Ranked among the leading causes of death among Filipinos in 2024 and 2025, diabetes affects millions of people and contributes to heart disease, stroke, kidney failure, blindness, nerve damage, and lower-limb amputations. Yet many of these complications can be delayed, or even prevented, through healthier lifestyles, early detection, appropriate medical care, and sustained disease management. 

Today, an estimated 4.7 million Filipino adults are living with diabetes, while roughly two in 10 have prediabetes. Prediabetes is a condition that can often be reversed through timely lifestyle changes but may progress to diabetes if left unmanaged. These figures underscore the urgent need to strengthen public awareness and encourage healthier habits before irreversible complications develop.

Elevated blood sugar, the hallmark of diabetes, also weakens the immune system, making people more susceptible to respiratory, skin, and urinary tract infections.

Because diabetes often develops silently, many people are diagnosed only after complications have already begun. Adults with risk factors, including overweight or obesity, hypertension, high cholesterol, a family history of diabetes, previous gestational diabetes, or advancing age should undergo regular blood glucose screening as advised by their physician. Early diagnosis allows timely intervention and substantially improves long-term health outcomes that avoid developing irreversible complications.

Preventing diabetes begins with addressing modifiable risk factors. Smoking and excessive alcohol consumption are among the major behavioural contributors to chronic diseases, including diabetes. Quitting smoking and limiting alcohol intake can significantly reduce the risk of developing noncommunicable diseases (NCDs) while improving overall health.

Equally important is adopting healthier eating habits. Frequent consumption of large portions of refined carbohydrates, including white rice, combined with physical inactivity, overweight or obesity, and genetic predisposition contributes substantially to diabetes risk. White rice remains a staple of the Filipino diet, with the Department of Science and Technology–Food and Nutrition Research Institute (DoST-FNRI) reporting that nine out of 10 Filipino households consume rice daily, averaging about four cups of cooked rice per household member each day.

White rice has a high glycaemic index, meaning it is digested rapidly and causes blood sugar levels to rise quickly after meals. During milling, much of its fibre, bran, and nutrients are removed, unlike brown rice, which retains these components and helps slow digestion and moderate blood glucose levels.

Rather than eliminating rice altogether, moderation and portion control are key. Replacing part of one’s intake with whole grains or lower-glycaemic carbohydrate sources such as brown rice, corn, sweet potatoes (kamote), cassava (kamoteng kahoy), and taro (gabi) can help achieve better blood sugar control. These foods release glucose more gradually into the bloodstream, making them valuable components of a balanced diet for diabetes prevention and management.

Portion size is just as important as food choice. The DoST-FNRI’s recommended healthy plate consists of one-half vegetables and fruits, one-third rice or other carbohydrate sources, and one-sixth protein-rich foods such as fish, tofu, chicken, pork, or beef. Preparing meals through steaming, boiling, stewing, baking, or air-frying instead of deep-frying further supports healthy eating.

Non-starchy vegetables such as moringa (malunggay), water spinach (kangkong), vine spinach (alugbati), cabbage, Chinese cabbage (pechay/bok choi), jute leaves (saluyot), and mustard (mustasa) are excellent choices because they are rich in fibre and naturally low in starch. Fruits also remain part of a healthy diet, although portion size matters because they naturally contain sugars. Avocados, guava, pomelo (suha), apples, pears, oranges, berries, and other fresh fruits can be enjoyed as part of a balanced meal plan.

Regular physical activity is another cornerstone of diabetes prevention and management. International and Philippine diabetes guidelines recommend at least 150 minutes of moderate-intensity aerobic exercise each week, complemented by two to three sessions of strength training. This combination improves insulin sensitivity, helps regulate blood sugar, supports healthy weight management, and lowers the risk of diabetes-related complications.

The benefits of exercise extend well beyond diabetes. Regular physical activity reduces the risk of heart disease, stroke, hypertension, several cancers, osteoporosis, and depression. It strengthens bones and muscles, improves flexibility, balance, and coordination, enhances sleep quality, boosts mood and cognitive function, and promotes overall well-being. Older adults and individuals with heart disease or other chronic conditions should consult their physician before beginning a new exercise program.

For people already diagnosed with diabetes, lifestyle modification should complement and not replace prescribed medications. Regular follow-up consultations, adherence to treatment, and routine monitoring of blood glucose, blood pressure, cholesterol, kidney function, and eye health are essential to preventing complications and maintaining a good quality of life.

Immunization is likewise an important but often overlooked component of diabetes care. Because diabetes impairs immune function, people living with the disease face a significantly higher risk of serious infections. Respiratory infections can trigger dangerous spikes in blood sugar that may require hospitalization. People with diabetes are twice as likely to die from influenza than those without underlying medical conditions and are more than four times as likely to develop pneumococcal disease. One study found that seasonal influenza vaccination reduced the need for flu-related medical care by 66% among people with diabetes, while pneumococcal vaccination has been shown to reduce hospitalization and death.

Creating healthier communities also requires supportive public policies. Safe spaces for physical activity, affordable access to healthy food, strong primary healthcare, smoking cessation programs, regular screening, and timely access to essential medicines all help people prevent and manage diabetes more effectively. A healthier Philippines depends not only on individual choices but also on systems that make healthy choices easier.

Diabetes is not inevitable, nor are its devastating complications. Through healthier lifestyles, early detection, adherence to treatment, regular physical activity, balanced nutrition, and preventive measures such as immunization, millions of Filipinos can live longer, healthier, and more productive lives. Reducing the burden of diabetes will require sustained commitment not only from individuals and families but also from communities, employers, healthcare professionals, and government working together to build a healthier future for every Filipino.

The Foods In Your Kitchen That May Influence Diabetes Risk

If you've ever wondered whether the foods you already eat could be working in your favour, new research suggests the answer might be yes.

A large, long-term study looked at whether people who eat more polyphenols (naturally occurring compounds found in everyday plant foods) are less likely to develop type 2 diabetes.

The results add meaningful weight to a growing body of evidence, and the top food sources might not be what you'd expect.

About the ELSA-Brasil polyphenol study

The research comes from the Brazilian Longitudinal Study of Adult Health (ELSA-Brasil), which followed 8,781 adults free of diabetes at the start for a median of 7.6 years.

Participants reported what they ate using a detailed food questionnaire at two points during the study, and researchers used an established nutrition database to estimate polyphenol intake across major classes: phenolic acids, flavonoids, stilbenes, and lignans.

Polyphenols are a broad family of plant compounds; more than 500 distinct types have been identified in the foods we eat. Beyond their well-known antioxidant properties, research suggests they may help the body manage blood sugar, respond to insulin, and keep inflammation in check.

With type 2 diabetes rates rising globally, understanding which dietary patterns offer real protection has become increasingly urgent.

Higher polyphenol intake linked to up to 27% lower diabetes risk

Over the course of the study, 1,453 participants developed type 2 diabetes. People who ate the most polyphenols overall were 19% less likely to develop the condition compared to those who ate the least.

When researchers looked at specific types, the associations got even stronger, with phenolic acids, hydroxycinnamic acids, flavonoids (including flavan-3-ols, flavones, and anthocyanins), and stilbenes each linked to a 13% to 27% lower risk.

There was another notable finding: people with the highest intakes of total polyphenols, phenolic acids, and stilbenes also showed smaller increases in insulin resistance over time.

That matters because insulin resistance (when your cells stop responding efficiently to insulin) is one of the primary ways type 2 diabetes develops. Slowing that progression is meaningful on its own.

The study didn't find significant links between polyphenol intake and fasting blood sugar or HbA1c, a longer-term blood sugar marker.

And because this was an observational study, it shows associations rather than direct causation; the large sample size and long follow-up period do lend the findings real weight.

What these results mean for everyday eating

The top contributors to polyphenol intake in this cohort weren't exotic superfoods.

Coffee accounted for nearly 40% of total polyphenol consumption, followed by red wine, yerba mate tea, orange juice, and oranges. These five foods alone made up the majority of polyphenol intake across the study population.

However, these rankings reflect the dietary habits of a Brazilian cohort, so the specific order may look different depending on where you live and what you regularly eat.

The underlying principle holds, though: polyphenols are concentrated in a handful of commonly consumed foods and beverages, and regular intake of even a few of them can add up meaningfully over time.

Easy ways to get more polyphenols from food

  • Coffee: The standout contributor in this study, accounting for nearly 40% of polyphenol intake. If you're already a daily coffee drinker, you may be getting more metabolic support than you realize.

  • Citrus: Orange juice and whole oranges were both among the top five polyphenol sources in the study, making them an easy, low-effort daily addition.

  • Yerba mate: Less familiar to some, but yerba mate tea ranked third in this cohort and is worth exploring if you want to diversify beyond coffee.

The takeaway

Polyphenol-rich foods were linked to up to a 27% lower risk of type 2 diabetes in a large, long-term cohort study, with higher intake also tied to slower progression of insulin resistance over time.

The most protective foods in the study weren't supplements or specialty items; they were coffee, citrus, and tea. Everyday dietary choices, made consistently, appear to carry real metabolic weight.
